To make a booking for accommodation or travel for the Bristol BI reunion weekend, or if you confirm or alter an earlier booking, please complete the form below and submit it to Sue Spence. You will get a reply by return.

As payment cannot be taken over the web, please complete this form, submit online by clicking the submit button and send a cheque for the deposit of £25 per person by post to the organiser:

This request:	I am making a booking Confirming/altering earlier booking My party comprises people. We intend to stay at Marriott Hotel Other hotel home with friends not decided for the weekend and expect to arrive in Bristol before Friday on Friday (Oct 11) on Saturday (Oct 12) on Sunday (Oct 13) (Please note that the ruenion organisers are not taking accommodation bookings. These should be made direct with hotels)
Organised events - Please reserve these places for my party	Saturday morning					1. I am interested in booking people for the tour of Harvey's of Bristol wine cellars (approx £12 each).
	Saturday afternoon					2. I am interested in booking people for the harbour trip with Bristol Packet Co (approx £5 each).
	Saturday afternoon					3. BI Golf tournament for people (green fees to be paid separately).
	Saturday evening					4. Reunion dinner, Saturday evening at £28 each including vegetarian option for people
	Sunday lunchtime					5. Traditional BI curry lunch at £20 each including non-curry option for people
Table plan and name badges	Please indicate your table seating requirements, if any, including all names and whether they are sending in separate booking forms. We will do our best to accommodate some, if not all, requests but I cannot guarantee to do so if it becomes over complicated. (Tables seat 10 people) Please list all those attending so that name badges can be provided. Mark those who have booked separately with *
Special interest groups and miscellaneous enquiries	Please use this space to inform Sue Spence if you wish to promote or present a film show or exhibition on a BI theme - give brief details, including location and timing here*. This space may also be used if you have any other requests or queries. *You should contact Sue Spence separately to discuss the arrangements, cost of equipment hire from the hotel.

On clicking submit request, this form will go to the reunion organiser. Don't forget to send your deposit cheque for £25 per person (payable to "BI Reunion" with name and address on the back) by post immediately to:

Your deposit of £25 per person includes the £7 booking fee. The final date for payment of the balances will be August 31, 2002 after which the organiser cannot guarantee to make any refunds, as the hotel requires payment in advance. Sue Spence will send out reminders in June 2002.
